What It Does
Stride Maintenance QR Forms lets you attach digital survey-style forms to equipment. Each form gets a unique QR code that you can print and post on the physical equipment. When a technician scans the QR code with their phone, they are taken to a secure web page where they can fill out the form — no Odoo login required.
This is useful for collecting inspection data, logging maintenance activities, or reporting issues directly from the shop floor.
Where to Find It
**Maintenance → Equipment** → open an equipment record → **QR Forms** tab

How to Use It
Creating a Maintenance QR Form
1. Open an equipment record in **Maintenance → Equipment**.
2. Click the **QR Forms** tab.
3. Click **Add a Line** to create a new form.
4. Fill in:
- **Form Name** — A descriptive title (e.g., "Daily Inspection Checklist").
- **Form Type** — Choose *Preventive* (scheduled inspection) or *Corrective* (reporting a problem).
- **Survey** — Select or create the survey/questionnaire to attach to this form.
5. Click **Save**.
Odoo automatically generates a QR code for the form.

Printing the QR Code
1. On the **QR Forms** tab, find the form you want to print.
2. Click the **Print** button next to the form.
3. A printable QR code label is generated. Post this on or near the equipment.

Scanning and Submitting a Form (Technician Workflow)
1. The technician scans the QR code posted on the equipment using their phone's camera.
2. A portal web page opens — no Odoo account or app required.
3. The technician fills out the form questions and submits.
4. A **Maintenance Request** is automatically created in Odoo with the submitted data linked to the equipment.

Reviewing Submitted Forms
- Submitted form responses are linked to the resulting Maintenance Request.
- Go to **Maintenance → Maintenance Requests** and open the request to view the form submission.
